One of the secret medals is earned for clicking the small face button in the bottom-center of the screen during the quiz, and another one is earned for right-clicking and selecting the "you cheater!" menu option. The last medal is received for clicking a hidden button on the menu screen. It's hard to describe exactly where it is, so I took a screenshot and circled the general area you'll need to click.
