Dunno. I think all of them get unlocked by beating the game in a specific mode.

So game has been updated. Invincible + medal aobs no longer work. Also a few new medals have been added, nothing too hard that can't be fixed without invincible. Phren you want to try and update that?

There are two additional medals that don't yet work:

And does anyone know how to kill a clown? I tried hitting them with the streamer but it doesn't seem to work and I can't think of any other way to damage them. Something related to the pies? I tried hitting them with a streamer too...
You don't actually have to hit the clown in any way. There is a part in the middle of the game where you stream from a block, and the block goes away, causing the clown to fall through and onto the red balls and die.

Nah I found it on my Story playthrough, it may not be there on Easy I think.

If you use this and fall into a pit you are fucked, but you can instead search for deaths as 4Bytes*1 and freeze it at 0. 2 values are found, I'll check and see if the medal still sends.
EDIT: Works along side the Invincible hack, and you will get the medal at the end for no deaths.
